I. OBJECTIVE
1. To provide a standard format on the creation of the Flow Charts of a manual
2. To provide guidelines in the creation of Flow Charts that is consistent throughout the department’s manual
II. DEFINITION OF TERMS
1. Flow Chart – a chart or diagram that shows the sequence of tasks or movements in a process or procedure.
III. GENERAL POLICIES
The Flow Chart must contain the following format:

Flow Chart Header, a block of text that appears at the top of every flow chart. It must reference the flow chart to the document where it comes from.
Process Owner, guides the user on the allocation of tasks involved in the process or procedure.
Activity, shows the flow chart of a process or procedure.
The Flow Chart must use the following symbols:
