III. GENERAL POLICIES

Qualified applicants must submit the following pre-employment requirements within 5 days:
• NBI Clearance
• Police Clearance
• Barangay Clearance
• Community Tax Certificate
• SSS ID/ Number (photocopy)
• TIN ID/ Number (photocopy)
• PAG-IBIG ID/ Number (photocopy)
• Transcript of Records/ Diploma
• Birth Certificate (Certified True Copy)
• Birth Certificate of Dependents (if applicable)
• Marriage Contract (if applicable)
• 1 pc. 1×1 picture with white background
• 1 pc. 2×2 picture with white background
• Employment Certificate (if applicable)
• Clearance from last employer (if applicable)
• Sketch of city or provincial address
• Medical result (original)

If the newly hired employee shall be assigned to outlet, the following additional requirements shall be submitted:
• Health Certificate from the municipality of the assigned outlet
• Mayor’s Permit to work from the municipality of the assigned outlet

For 201 file purposes, the Recruitment Officer shall ensure that all requirements submitted by newly hired employee of the Support Group (or Operations Group) are complete. These shall be forwarded to the Personnel and Administration Officer.
